RELEASE OF LIABILITY
We______________________, ________________________, hereby release and hold harmless from all legal litigations Roberta S Barclay, Elizabeth A & Christopher M Smith, Barclay Farms, their heirs and assigns and their current insurance company or companies of any and all liabilities that might occur while we and/or our children are attending, and/or participating in activities on the property known as 4713 & 4715 State Route 82 NW, Newton Falls, Ohio 44444.
By signing this document I fully understand and fully release and hold harmless from any and all legal litigations the above parties named Roberta S Barclay, Elizabeth A and Christopher M Smith, Barclay Farms, their heirs and assigns and their current insurance company or companies of any and all liabilities that might occur on the above named property on this ________day in the month of_____________200__. This agreement shall remain in effect until December 31, 2030.

Adult visitors signature for minor child is
required and signifies that the adult signing for such child is
purporting to be the legal guardian/custodial parent and as such is
totally responsible financially and otherwise for any or all
liabilities that might occur during such visits/activities as may take
place while this release is in effect.
